The patient is ultimately responsible
to know their insurance policy and to provide all necessary
insurance information. If there are any questions the patient can
contact member services or the insurance coordinator at their work.
Patients are responsible for co-pays at the time of service.
For your convenience, Hershey Physical Therapy Service will bill
your insurance company. If you participate in an HMO insurance plan
you will need to obtain a referral from your referring physician.
Your health insurance is a contract between you and your insurance
company. However, plans vary widely with deductibles and
co-insurance. As a courtesy we will attempt to clarify the specifics
of physical therapy requirements prior to initiating treatment.
