Your Goals are
Our Goals
It
is extremely important that you, your physician, and the therapist
all agree with what we are trying to accomplish. We want to work
with you in an effort to resolve your current condition so that you
can function at the best of your ability at home, work, school, or
while engaged in leisure time activity. While we intend to solve
your immediate needs by improving your condition or relieving pain,
we are primarily concerned with the long-term outlook for your
disability or injury; that it does not return or become worse.
Teaching you about your problem is one of our primary
responsibilities and understanding that responsibility will help us
in working together.

Conclusion
of Therapy
When you achieve
the objectives of your care plan, or when maximal benefit from
therapy has been reached, you will be discharged. A discharge
evaluation will be performed to document your status at final visit.
Your last visit is not the end of your therapy. We will expect you
to use what you have learned, to avoid re-injury, and to regularly
perform home exercise. During the course of your therapy we will
strive to provide a level of service so that should the need arise,
you will happily return to our practice or refer family and friends.
